TRIAL REGISTRATION

The Parent/Guardian acknowledges that the activities carried out during Bryst Football Academy training carry with them an interit risk of personal injury, property damage and loss or theft of personal property, and the execution of this document acknowledges and confirms that he or she is well aware and understands these risks. The Parent/Guardian further warrants that his/her participant is in good physical and/or mental health and is able to freely participate in the soccer activities and any health restrictions have been noted. The Parent/Guardian agrees that he/she is solely responsible and will indemnify APEX Soccer from and against all liability whatsoever resulting from personal injury, loss or theft,
howsoever caused. The Parent/Guardian acknowledges and agrees that Bryst Football Academy reserves the sole and exclusive right to use any photographs and/or videos taken during the program for advertising and/or instructional purposes, without cost or charge and the Parent/Guardian consents to photographs or videos being taken for the purposes contained herein.
